VAT exemption form
To get a VAT Exemption Form, visit
www.hmce.gov.uk and choose 'Public Notices' on the left hand side. Once in that section select Public Notice 701/7. Public Notice 701/7 is about Tax Exemption for Disabled people and on page 21 item 10.1 is the Eligibility Declaration by a Disabled Person.
You can print off the form and then they suggest you use the wording with additional space to write about your disability on another sheet of paper to send to us.
